The statement of fitness for work (Arbeitsunfähigkeitsbescheinigung) consists of three or four parts. If you doctor's practice transmits the details on your fitness for work digitally to us, we receive this statement directly from the practice. Your doctor then provides you with copies for your records and for your employer.

However, if your doctor is not yet participating in this new electronic transmission procedure or should this not work one time, your doctor will also provide you with the certificate "zur Vorlage bei der Krankenkasse" [for submission to the health insurance fund]. The statement of fitness for work also comprises:
- The certificate labelled "zur Vorlage beim Arbeitgeber" [for submission to the employer]: this is intended for your employer or the employment agency (does not include any diagnoses on it).
- The certificate labelled "Ausfertigung für Versicherte" [copy for insurees]: this is the copy for your records.
- The certificate labelled "Ausfertigung zum Verbleib beim Arzt" [copy for physician’s records]: this part is for your doctor to keep (includes diagnoses on it).
